本片是根据一个真实的故事改编的，奥斯卡奖获得者罗宾威廉姆斯在本片中扮演医生帕奇亚当斯，他曾在《早安，越南》《清醒》等片以及最近获奖的影片《心灵猎手》（Good Will Hunting）中成功地扮演了不同的角色，而在本片中他成功塑造了一位感人的医生。
帕奇亚当斯在学校中总是因为“过分乐观”而受到责备，曾经有一个导师对他说：“你想要成为小丑，那就去马戏团。”
其实，帕奇亚当斯是想成为一个小丑，但同时他也想成为一个医生。于是他将两者综合，成为了现在的他。
帕奇亚当斯在六十年代末进入弗吉尼亚医学院学习，毕业后，他成立了一个学院，致力于更接近的人性化的治疗方法，八十年代中期他的个性化的诊所受到了媒体的注意并由此出版了一本关于他成就的书。在书中，亚当斯阐述了他的基于幽默的处方，解释了他为什么穿得象个大猩猩，为什么在病房里堆满气球或在浴缸里装满蜡烛以引起欢笑，以给病人精神上的接近或纯粹的欢笑。在使用特殊的方法来减缓病人的紧张情绪后，亚当斯以先驱者的身份提出了医生应该治疗人，而不是疾病，他坚信，同情、加入和心灵相通对医生来说与灵丹妙药和技术进步一样重要。在电影中，开始没有多少人赞成亚当斯的观点，学院院长瓦尔考特尤其反对他的方法，而亚当斯的室友米奇也认为他是一个孩子气的家伙。但同时他也影响了一些人，包括护士乔蕾塔、同学楚门和卡琳。亚当斯发现他在儿童病房大受欢迎，因为他的可笑、古怪的方式驱散了儿童们的恐惧感。
在对传统方式的抗争中，他疏远了一些人、使更多的人吃惊，但是他看到了自己的价值。在不断的追求和探索中，亚当斯实现了自己的梦想－开了一个诊所，每个人在这里不仅遇到了医生，还遇到了朋友......

《妙手情真》中的亚当斯与《星际旅行》中的宇航员们都是在不断的探索着世界，只不过《星际旅行》中在探索的大主题是宇宙是未知，而《妙手情真》中探索的则是人性是爱，看似没有多大联系，其实确都是情感的交织。《星际旅行》中由大的世界观而逐渐深入到对人性人情层面的探索中，这是以大见小；《妙手情真》中的亚当斯则以一个微不足道的小角色向我们诠释了一种对爱的信仰和对理想的追求的人性魅力，可谓是以小见大，都有异曲同工之妙......
这首“Faith of the Heart”心的信念，在《星际旅行之进取号》中被改编了一下，标题也换成了“Where My Heart Will Take Me”，下面是原作与改编作品的对比......
